Axis Technologies, Inc. (ATI) warrants to the purchaser that its electronic fluorescent ballasts will be free from defects in material and workmanship as conditioned herein for a period of five (5) years from Date of Installation (or Date of Manufacture if installation date is not known). If the ATI ballast fails to operate during the warranty period, ATI will provide replacement ballast and include a $10.00 provision for ballast installation costs.

This warranty is not applicable and is voided to any product manufactured by Axis Technologies, Inc. which is not stored, installed, operated and maintained in accordance with:

· The Standards for Safety of Underwriters Laboratories, Inc. (UL)
· The Standards for the American National Standards Institute (ANSI)
· The National Electric Code (NEC)
· The Specific Instructions Provided by Axis Technologies, Inc. for the Installation of the Product(s)
· Accepted industry practices.

This warranty will not apply in the event of conditions demonstrating abnormal use or stress, such as case temperatures in excess of 70 °C, over/under voltage conditions, excessive switching cycles, defective lamp sockets, or improper installation.

The warranties set forth herein are in lieu of any and all other warranties expressed or implied including the warranties of merchantability and fitness for a particular purpose. In no case shall Axis Technologies, Inc. be obligated or liable for any indirect, consequential, or incidental damages for breach of this or any other warranty, expressed or implied, whatsoever.

Should at any time within the stated five year period from Date of Installation (If registered, Date of Manufacture if not registered) any appearance of defect in material or workmanship appear, purchaser must notify Axis Technologies, Inc. by calling 866/458-9880 or 402/476-6006. Written notice may be mailed to:

As a condition to qualify for ballast (no allowance for lamp) replacement under the provisions of this limited warranty, defective ballasts must be available for return to Axis Technologies, Inc. at its request for inspection and test analysis. Prior written approval for return must be obtained from ATI. ATI reserves the right to examine all failed ballasts and reserves the right to be the sole judge as to whether any ballast are defective and covered under this warranty.

Complete the Axis DH&H Dimming Electronic Ballast Registration Form and mail originals to our mailing address below or fill-out our online registration form at www.axistechnologyinc.com. Completed registrations received within 30 days of installation by Axis Technologies, Inc will activate the 5-Year Limited Warranty.
